The Homecare Assessment Reablement Team (HART) service helps individuals develop the confidence and skills they need to live as independently as they can at home and in their local community. The team works closely with health partners to deliver an integrated therapy offer and achieve the most independent outcome for each person. Care Act assessments are completed (where necessary) to reduce hand-off to other teams and ensure the assessment is completed in a timely manner. In addition to this, HART Urgent provides an intensive short-term personal care and reablement service for people who are at high risk of deterioration and require urgent social care support. The overarching service operates on a 24/7 basis and is the departments main contact point outside of usual business hours. This post ensures that referral activity is progressed and managed in a timely manner and that all available capacity is maximised.
