Job summary

Penwith Primary Care Network is seeking a Pharmacy Technician to work with GP practices across West Cornwall. The role focuses on medicines optimisation, patient-facing activities and collaboration with GPs, nurses and the broader multidisciplinary team to improve safe, cost-effective prescribing.

Qualifications

• Qualified pharmacy technician registered with GPhC
• Minimum of 2 years' experience in primary care
• Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
• Evidence of continued professional development (CPD)

Responsibilities

• Support the pharmacist to provide a clinical pharmacy service in general practice.
• Participate in practice meetings to improve integration of the pharmacy technician within general practice and promote prescribing optimisation.
• Undertake patient-facing and patient-supporting roles to ensure effective medicines use with shared decision-making.
• Carry out medicines optimisation tasks including administration, reviews and medicines reconciliation; support patients in using medicines effectively.
• Support medication reviews and reconciliations for new care home patients and synchronising medicines for transfers between care settings with local community pharmacists.
• Provide specialist expertise to address public health and social care needs, including lifestyle advice and tackling health inequalities.
• Take a central role in shared care protocols and liaise with specialist pharmacists for complex patients.
• Support antimicrobial stewardship initiatives to reduce inappropriate antibiotic prescribing.
• Assist in medicines optimisation schemes and patient safety audits; support implementation of national prescribing policies within GP practices.
• Develop relationships with other pharmacy staff to support integration of the pharmacy team across health and social care.

Education

GPhC registration

BTEC/NVQ Level 3 or equivalent in pharmaceutical sciences

CPD ongoing professional development

2+ years' experience in primary care
