Job summary

Polden Medical Practice is a stable, well-established rural dispensing training practice serving about 11,000 patients across two sites in Somerset. We seek a GP Partner to join our partnership, working 4–8 sessions weekly and taking an active role in leadership, development and future direction.

This role offers a genuine pathway to partnership within a supportive teamwork culture, with flexible working and opportunities to contribute to service improvement and teaching within a Bridgwater Bay

Qualifications

Looking for a Role That Still Feels Like General Practice? Polden Medical Practice is a thriving rural dispensing training practice in Somerset serving approximately 11,000 patients across two owned sites. We are looking for a GP Partner who values continuity and teamwork. We combine traditional continuity of care with modern systems that support safe, efficient and sustainable general practice, keeping patients at the heart of everything we do. Situated close to Junction 23 of the M5, we're within easy commuting distance of Bristol and Taunton while offering the benefits of rural practice and a strong sense of community.

Main duties of the job:

Sessions are flexible and can range from 4 to 8 sessions per week. Our multidisciplinary team creates a supportive environment where workload is shared appropriately, and clinicians can focus on delivering high-quality patient care. We operate a modern digital access model supported by efficient administrative systems and a commitment to reducing unnecessary workload wherever possible.

Main duties of the job
Clinical:
  • Provide comprehensive primary care services to registered and temporary patient
  • Undertake routine, acute and complex consultation
  • Diagnose and manage acute and chronic medical conditions
  • Deliver evidence-based care in accordance with national and local guidance
  • Manage patients with long-term conditions and contribute to QOF achievement
  • Prescribe safely and effectively within professional guidelines
  • Participate in same-day access and triage services
  • Maintain accurate and contemporaneous clinical records
  • Make appropriate referrals to secondary care and community services
  • Work collaboratively with the wider multidisciplinary team to ensure high-quality patient care
Partnership and Leadership:
  • Take an active role in practice leadership and decision-making
  • Contribute to the strategic direction and future development of the practice
  • Participate in partnership and PCN meetings and governance processes
  • Help shape service development and quality improvement initiatives
  • Support the delivery of safe, effective and sustainable primary care services
  • Participate fully in the management and running of the business
  • Work collaboratively with partners, managers and the wider practice team to ensure organisational success

Job Title: GP Partner Hours: 4-8 sessions per week (flexible) Location: Polden Medical Practice, Edington and Woolavington, Somerset Responsible to: GP Partners Contract Type: GP Partner appointment subject to a 12-month mutual assessment period, following which progression to full equity partnership will be considered by mutual agreement.

We are seeking an enthusiastic and motivated GP to join our partnership team for 4-8 sessions per week. The successful candidate will provide high-quality, patient-centred general medical services while taking an active role in the leadership, development and future direction of the practice. This role offers the opportunity to become a genuine long-term equity partner within a stable and supportive partnership structure.

Main Duties and Responsibilities
Clinical Responsibilities
  • The post holder will:
  • Provide comprehensive primary care services to registered and temporary patients
  • Undertake routine, acute and complex consultations
  • Diagnose and manage acute and chronic medical conditions
  • Deliver evidence-based care in accordance with national and local guidance
  • Manage patients with long-term conditions and contribute to QOF achievement

Same Day Access and Triage
  • The practice operates a fully digital same-day access system using Rapid Health
  • The post holder may be required to:
  • Review and clinically prioritise same-day patient requests
  • Support safe and effective patient navigation and care pathways
  • Work closely with Advanced Nurse Practitioners and the wider clinical team
  • Ensure appropriate escalation and management of urgent clinical presentations
  • Contribute to effective demand management within the practice
Partnership, Leadership and Business Responsibilities
  • The post holder will:
  • Take an active role in practice leadership and decision-making
  • Contribute to the strategic direction and future development of the practice
  • Participate in partnership meetings and governance processes
  • Help shape service development and quality improvement initiatives

Person Specification
Essential
  • Fully qualified General Practitioner
  • GMC registration with licence to practise
  • Inclusion on the NHS Performers List (or eligible)
  • MRCGP or equivalent qualification
