Job description

Surbiton PCN LTD are looking for a proactive and confident part-time GP Assistant to join our friendly team. The successful candidate will work at Brunswick Surgery for 12 hours per week (Surbiton Health Centre) as part of a job share and will play a key role in supporting the management of patients with long-term conditions, including hypertension, diabetes, and other chronic illnesses.

  • Brunswick utilise EMIS Web clinical system and have an excellent QOF achievement.

Main duties of the job

  • Previous experience in a healthcare setting.

The Surbiton & Chessington PCNs Ltd includes Surbiton Health Centre PCN and Chessington & Surbiton PCN, which work together through a limited company with a joint Board: Surbiton & Chessington PCNs Limited.

The PCNs have 3‑ and 5‑member GP practices respectively, and together account for a total registered population of close to 66,000.

The networks have diverse communities including commuter areas, young families, deprived pockets, and frail elderly / care homes.

The PCNs vision is to help to maintain and improve the health and healthcare for our local populations, and the sustainability of our member GP practices, in line with wider NHS strategies and plans.

Job responsibilities

Key Responsibilities:

  • Assist in the coordination and management of patients with long‑term conditions (including hypertension, Diabetes and chronic illnesses.
  • Book patients for reviews with GPs and the nursing team.
  • Ensure timely follow‑up and accurate record‑keeping.
  • Provide excellent patient support and communication throughout their care journey.
  • You will act as a key liaison between the admin team, NHS services and patients to promote safe and efficient care
  • Assist in the management and uploading of information relating to safeguarding documentation and audit.
  • Maintain and monitor emails relevant to the GP and patient care.
  • Sorting all clinical post and prioritising for the GP in terms of actions.
  • Signposting some post to others such as clinical pharmacist etc.
  • Monitoring and assisting the GPs in managing, prioritising, and actioning tasks.
  • Extracting all information from clinical letters that needs coding and adding to notes.
  • Preparing patients prior to going in to see the GP ensuring the patient has everything they need and is aware of the need to provide accurate and concise information to the GP.
  • Completing basic (factual) forms and core elements of some forms for the GP to approve and sign such as insurance forms, mortgage, benefits agency forms etc.
