CHOC PCN are looking for an enthusiastic and motivated GP Assistant who shares our values and is committed to providing excellent care, to join our growing team.

As a GP Assistant you will support our GP practices and work closely within the PCN Same Day Access Service, which provides on the day acute care to the patient population. You will be working closely with our ANP / ACP and paramedic practitioner team. The successful candidate will share our values and have the energy, skills and enthusiasm to contribute to shaping how we support our patients. You will be required to act within your professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and a Primary Care Network.

To meet the requirements of the Same Day Access Service, the post holder will be required to work Monday to Friday from 8am in the morning.

This role is part of the PCN Additional Roles Reimbursement Scheme.

Main duties of the job

Role Overview:

  • Support smooth clinic operations by performing routine admin and clinical tasks for the PCN and Practices.
  • Assist with day-to-day SDAS operations, including patient queries, rotas, and admin support.
  • Develop and update appointment templates for SDAS and PCN services.
  • Arrange appointments, referrals, tests, and follow-ups.
  • Complete simple clinical observations (e.g., urine dipstick, BP, phlebotomy) as directed.
  • Help SDAS liaise with outside agencies and secondary care for emergency admissions.
  • Support immunisation campaigns, MDT meetings (palliative care, complex families, mental health, drug & alcohol), and practice QOF reviews.
  • Track experience against the GPA competency framework and undertake required training, including annual appraisal.
  • NVQ/Apprenticeship Level 2 Business & Admin (or equivalent)
  • Excellent IT skills; experience with EMIS, Docman, AccuRx, MS Office

We are 4 GP Practices in Congleton and Holmes Chapel: Holmes Chapel Health Centre, Lawton House Surgery, Meadowside Medical Centre, and Readesmoor Medical Centre. Together as a PCN, we serve around 50,000 patients, providing General Practice services. By working with local care providers, we improve the patient experience of primary care. We are based in the Cheshire East Council area.

Vernova Healthcare CIC is the employing body for CHOC PCN Additional Role staff. Vernova is owned and supported by all GP practices in Eastern Cheshire.

The successful candidate will join a growing PCN Team (including Clinical Pharmacists, FCPs, PAs, SPLWs, ANP, Paramedic Practitioners, and MHPs) and will be supported by the PCN Operational Manager and Digital & Transformation Lead. Regular development sessions, supervision, training, and opportunities to enhance skills will be provided, linking with the PCN Clinical Educational Lead as appropriate.

We’re looking for an enthusiastic and motivated GP Assistant to play a key role in our Same Day Access Service (SDAS), which provides urgent care to our patient population. You’ll help keep clinics running smoothly, support clinicians, and make a real difference to patient experience.

What You’ll do
  • Assist with day-to-day operations of SDAS, including patient queries and scheduling.
  • Perform routine clinical tasks (e.g., blood pressure checks, phlebotomy, urine dipsticks).
  • Coordinate referrals, tests, and follow-up appointments.
  • Support immunisation campaigns and multidisciplinary team meetings.
  • Help manage patient feedback and quality improvement initiatives.
  • Contribute to education by supporting student placements across practices.

Person Specification
  • 5 GCSEs, ideally at A-C, but equivalent level of work experience will be considered.
  • Experience of using software packages such as EMIS, EMIS Enterprise, Docman, AccuRx and Microsoft Office.
  • Experience working in Primary Care.
  • Good communication and inclusive telephone and inter-personal skills.
  • Organised, good attention to detail.
  • Excellent customer care focus.
  • Ability to handle demands with professionalism, tact and diplomacy.
  • Work as part of a team to support the overall delivery of the team/department.
