A regional health service organization is seeking motivated healthcare professionals to join their Continuing Healthcare team in Manchester. This role involves managing a caseload, conducting person-centred assessments for NHS Continuing Healthcare, and ensuring compliance with national standards. Candidates should have relevant health professional qualifications and the ability to work within a collaborative, multi-agency environment.
Greater Manchester Integrated Care is a fully authorised ICB responsible for the full range of ICB statutory duties and powers. Each Place Based Team within NHS GM aims is to improve the health of the people and reduce health inequalities through strong, clinically led commissioning of high‑quality healthcare services. The GM Integrated Care Partnership are helping organisations work better together with people and communities. We want people across all our boroughs to stay well, whatever their age, and whatever issues or conditions they might be managing. We're bringing together all the different organisations that support people's health and social care, so that services can join up and act faster when people need support. Integrated care means starting with the person, understanding they will have different needs as they move through life, and connecting them with the right care. This new NHS organisation, overseen by a Board, is in charge of the NHS money and making sure services are in place to put plans into action. Made up of representatives from the NHS and the local council. The partnership operates at three levels: neighbourhood, locality and Greater Manchester and has a single vision and strategy.
The role sits in the Continuing Healthcare & Individualised Commissioning team which is part of a wider Directorate led by the Associate Director of Nursing, Quality and Safeguarding.
An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced band 5 RMNs, RGNs or Allied Health Care professional who can work autonomously and collaboratively in a multi‑agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person‑centred assessments so CHC and FNC eligibility can be determined.
The successful post holders will have the opportunity to work within a successful, progressive Integrated Care organisation and make a positive contribution to the lives of people in Greater Manchester. Working within the remit of the National Framework for Continuing Healthcare, the post holders will deliver an effective and competent level of service, consistently delivering a client‑focused service. The post holders will assist and support in the facilitation of patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded‑Nursing Care is implemented and complied with, according to the principles and processes.
