Job summary

Aneurin Bevan University Health Board (ABUHB) is recruiting an experienced nurse to provide person-centred therapeutic support to older people with mental health needs across community settings, care homes, and memory services.

You will carry out specialist assessments, reviews, and interventions for dementia, cognitive impairment, and other mental health conditions, ensuring care is evidence-based and least restrictive.

Qualifications

• Registered with the NMC and eligible to practice in the UK.
• Experience in older adult mental health or memory services is desirable.
• Ability to discuss with patients, carers and staff with empathy and professionalism.

Responsibilities

• Provide person-centred therapeutic support to older people across community settings, care homes, and memory services.
• Carry out specialist assessments, reviews and interventions for dementia, cognitive impairment and other mental health conditions; ensure care is evidence-based and least restrictive.
• Work flexibly across services; support patients, carers and staff while contributing to service development and quality improvement.
• Help plan and oversee care for patients in primary and secondary care, including those living in the community and in care homes.
