The role also carries medical leadership, clinical record-keeping and active involvement in clinical governance. What the job involves

• Provide clinical assessment and diagnosis, making timely decisions on future management
• Provide medical leadership
• Provide care and services to patients of all ages and levels of need
• Act as a specialist resource for healthcare staff, patients and carers
• Maintain clinical records, written or computer-based, using READ and other coding systems
• Liaise with relevant agencies and services to promote awareness of the service
• Give health education and advice to patients and carers to support self-management
• Work with the Clinical Lead and team to embed clinical governance in day-to-day practice
• Take an active part in clinical governance and effectiveness activities
• Communicate with patients and carers to ensure effective care delivery

Qualifications

Experience
