The post holder will act as a senior clinical decision maker, working within a dynamic multidisciplinary team to assess, diagnose and manage acutely unwell patients in the community, while contributing to service development, education, governance and quality improvement.

The @Home service delivers acute clinical care to patients in their own homes, aiming to prevent hospital admissions and facilitate faster discharges. The service offers a 2 hour Urgent Community Response (UCR), as well as same-day and next-day interventions.

Referrals are received from hospital wards and also colleagues in primary care, ambulance services, NHS 111, and other community-based health and social care providers. The care and interventions are provided by a multidisciplinary team including Doctors, Nurses, Occupational Therapists, Physiotherapists, Nursing Rehab Support Workers, Pharmacists, and Social workers. The team works collaboratively to create individualised care plans, delivering clinical assessments, IV medication administration, and management of acute exacerbation's of chronic illnesses. Remote monitoring is also provided within the service, allowing for early detection of deterioration and ongoing symptom management through the use of technology. The service also supports patients by addressing sudden declines in physical function, falls, or social support needs that may otherwise lead to hospital admission.

Main duties of the job

As work is community centred the post holder must be able to travel throughout Lambeth and Southwark boroughs.

  • To assess, diagnose, treat and refer patients seen in their own homes
  • Support multidisciplinary clinical decision-making
  • To action test results appropriately and make necessary onward referrals
  • To manage patients with a multi-disciplinary team approach
  • To attend clinical meetings, including handover and multi-disciplinary meetings

Provide clinical support and advice to the multi-disciplinary team

Deal with urgent clinical queries or safeguarding issues

To provide oversite and support to resident medical staff.

All medical staff are expected to take part in clinical governance activity, including quality improvement, clinical audit, clinical guideline and protocol development and clinical risk management. They will be expected to produce evidence of their contribution in these areas and their audit of their own clinical work as part of their annual appraisal.

Job responsibilities

Person Specification
Clinical Governance, Management and IT
  • Knowledge of and involvement in Clinical Governance issues including incident reporting
  • Proficiency with relevant IT systems, good keyboard skills
Risk and Audit
  • Understand principles of audit and quality improvement and evidence of contribution to effective quality improvement work
Qualifications and Training
  • Full UK GMC Registration and a licence to practice

  • Demonstrate an understanding of the application of evidence to practice in working in Hospital at Home services
  • Demonstrate an understanding of the application of evidence to practice in working with a Interest in Hospital @Home services/Urgent care response / remote patient monitoring
  • Defined track record of research, publications/ understanding / knowledge of research ethics
  • Experience of teaching and training undergraduates, postgraduates and junior medical staff
