Job Summary

We have an exciting opportunity for a permanent, Part‑time General Practice Assistant. Are you an experienced Health Care Assistant wishing to make your next career move at our forward‑thinking GP Practice, based at Hutton Rudby Surgery with cover at Mowbray House Surgery? The hours will be Monday, Wednesday and Thursday.

Main Duties

Supported by the nursing and general practitioner team within the practice, the GPA will deliver care within the boundaries of their role and in accordance with national, local and specific practice guidelines and protocols. Responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making within the parameters of their qualifications and role requirements. They will work collaboratively with the general practice team to meet the needs of patients and support the delivery and procedures and providing GPA guidance as required, working collaboratively with the general practice team to meet QoF and IIF targets. They will focus on supporting the patients with chronic, long‑term, complex health needs, providing a comprehensive review of the patients overall health including physical, emotional and physiological support. The post holder will make decisions, provide safe, evidence‑based, cost‑effective, proactive, individualised patient care.

Job Responsibilities

Clinical
  • To undertake all duties inline with the training and instruction provided by the general practice team.
  • Responsible for the management of patients with LTC(s) by ensuring the smooth running of chronic disease clinics, including clinical, emotional, physiological reviews and support.
  • Responsible for clinical and administrative pathways to provide patients with continuity, coordinated, informed, planned care.
  • Assist patients with their medication, compliance and check any equipment used, such as inhalers, are working effectively.
  • Responsible for QoF and IIF targets and maintain a high level of achievement.
  • Undertake routine tasks related to patient care, including but not exhaustive:
  • Venepuncture
  • Blood pressure monitoring
  • 12‑lead ECGs
  • Carry out baseline observations such as pulse oximetry, temperature and pulse rate
  • Height, weight and BMI measurement
  • Removal of sutures/clips
  • Basic wound care/dressings
  • The testing and result recording of clinical specimens, including urinalysis
  • New patient health checks
  • Vitamin B12 injections
  • Influenza and Pneumonia vaccinations
  • Chaperone
  • Ensure specimens are recorded and ready for onward transportation including blood centrifuge
  • The ongoing cleaning of clinical equipment and working surfaces
  • Ensure adequate infection control procedures in consulting rooms
  • Participate in any relevant health promotion and screening activities
  • Maintain accurate and contemporaneous records
Administration
  • Assist with the call/recall system for LTC management
  • Arranging appointments, referrals, tests and follow‑up appointments as required
  • Work closely with the care coordinator and social prescribers
  • Maintain the store and treatment room, ensuring it is clean and tidy
  • Assist with monitoring and recording of fridge temperatures; ensuring fridges are cleaned routinely in accordance with guidelines
  • Attend and participate in any practice meetings when required
Communication

  • Assist in promoting and maintaining health, safety and security as defined in the practice Health and Safety Policy, including using personal security systems, identifying risks, managing those risks, updating knowledge and skills, using appropriate infection control procedures and maintaining tidy, safe work areas
