Frequent Service User Support Worker

We are looking for an enthusiastic, motivated individual with a background in nursing, mental health, allied health, or relevant experience managing complex caseloads to provide holistic advice and support to adults who frequently use urgent care services in West Kent.

Our friendly team of health practitioners has extensive experience supporting adults, children, and families through life's challenges. You'll join a nationally recognized team that is passionate about their work and supportive of one another.

This service provides strong support for people who frequently access health services, including A&E, primary care, and mental health services.

Main duties of the job
  • Collaborate in a multi-disciplinary approach across various providers.
  • Identify the unaddressed medical, psychological, and social needs of patients who frequently use urgent care services.
  • Provide listening and non-judgmental support to patients’ concerns.
  • De-escalate crises related to physical or mental health.
  • Act as a bridge between patients and community services to help prevent future crises.
