A community health service in Coxheath is seeking an enthusiastic individual to support adults who frequently use urgent care services. The role involves managing complex caseloads, collaborating with various providers, and addressing both medical and psychological needs. The ideal candidate will have a background in nursing or mental health and be passionate about delivering high standards of care. We are looking for an enthusiastic, motivated individual with a background in nursing, mental health, allied health, or relevant experience managing complex caseloads to provide holistic advice and support to adults who frequently use urgent care services in West Kent.
Our friendly team of health practitioners has extensive experience supporting adults, children, and families through life’s challenges. You'll join a nationally recognized team that is passionate about their work and supportive of one another.
This service provides strong support for people who frequently access health services, including A&E, primary care, and mental health services.
This exciting role within a successful service offers the chance to be creative and influence how services are delivered.
You will collaborate in a multi-disciplinary approach across various providers. The role involves identifying the unaddressed medical, psychological, and social needs of patients who frequently use urgent care services. These needs will be met through different methods, such as listening to patients’ concerns without judgment, de‑escalating crises related to physical or mental health, and acting as a bridge between patients and community services to help prevent future crises.
