Job summary

East Cornwall PCN is seeking an experienced Frailty Practitioner to support our reactive frailty service. The role involves patient-facing work across Saltash/Torpoint and Liskeard/Looe areas, with a hybrid pattern between home, GP practices and other health locations.

Regular travel within the locality is required. You will assess, intervene and coordinate care for people living with frailty, aiming to prevent hospital admissions and support safe return home after discharge.

Qualifications

• Substantial post-registration experience with frailty, multimorbidity or complex needs.
• Experience of holistic assessment, care planning and risk management.
• Ability to travel independently across designated areas and undertake home visits.
• Registration with NMC (nurse) or HCPC (OT/PT) is required.

Responsibilities

• Provide timely assessment, intervention and care coordination for people living with frailty.
• Coordinate care across GP practices and multidisciplinary teams.
• Support discharge planning and avoid avoidable hospital admissions.
• Travel to home visits across Saltash & Torpoint, and Liskeard & Looe areas.
