This post sits within the multidisciplinary frailty team of the Cheltenham Peripheral Primary Care Network, which links five GP practices, and works alongside the existing frailty nurse. The role centres on helping frail, older or vulnerable people in the community stay independent at home through proactive care planning and planned transfers between care settings. What sets it apart is its reach across primary, secondary, social care and voluntary sector partners, using a strengths-based approach. Candidates need current NMC registration as an adult nurse and must be able to practise autonomously.
