Job description
Role Overview

The Frailty Team at Frimley Park Hospital are seeking an organized, proactive team player to join the Frailty Team as the administrator for 25 hours a week on a 12‑month fixed term contract. This patient‑facing role supports daily administrative tasks in the Same Day Emergency Care Department and handles data collection and analysis related to team performance.

Responsibilities
  • Manage administration of the Frailty Liaison Team and the services the team provides.
  • Support daily administration and flow of the Frailty Same Day Emergency Care (SDEC) Department, including becoming a super‑user in EPIC to support the team with EPIC queries.
  • Act as a point of contact on the SDEC for any patients or visitors, answering phone calls and relaying messages to the appropriate clinician.
  • Use EPIC to track patients onto the SDEC to support accurate patient location and data records, and to support the admitting and discharging of patients from the SDEC.
  • Manage the Frailty Team’s records and databases, including developing, using, and maintaining databases and spreadsheets containing patient details and outcomes.
  • Ensure accuracy of patient records, updating them with new or corrected information as required.
  • Share information/documentation with community partners as necessary, within data protection guidelines.
  • Produce and send letters to GPs and follow‑up with patients who have been discharged from hospital.
