Frailty Advanced Nurse Practitioner – Leadership Role – Plymouth

An exciting opportunity has arisen for an Advanced Nurse Practitioner (ANP) with a specialist interest in frailty to join a progressive, patient-focused healthcare team in Plymouth. This is a senior, leadership-level position, ideal for an experienced ANP looking to shape and develop frailty services within a collaborative, forward-thinking environment.

The role is a mix of clinical leadership and hands-on patient care, focusing on supporting frail and complex patients both in the community and in local care homes. You’ll need to be a confident driver, as travel between sites and care homes across the Plymouth area will form a key part of your day-to-day work.

Working within a well-established primary care network, you’ll lead on the coordination and delivery of frailty pathways — collaborating closely with GPs, pharmacists, paramedics, and community teams to deliver proactive, person-centred care. As part of the leadership team, you’ll help shape local frailty strategy, mentor colleagues, and drive service innovation across the network.
