Job summary

Frimley Health NHS Foundation Trust is seeking an experienced Frailty Advanced Clinical Practitioner to join the Integrated Frailty Liaison Team on a fixed-term basis until June 2027. The ACP will spend at least 90% of their time in direct autonomous clinical practice, managing a cohort of patients with complex frailty needs and working across acute, community and primary care settings.

Responsibilities

• Spend the majority of time (≥90%) in direct autonomous clinical practice, managing a cohort of patients with frailty and working across organisational boundaries to ensure seamless care.
• Provide clinical leadership and support to colleagues, helping to shape and develop the Frailty service in line with best practice.
• Undertake comprehensive specialist holistic assessments for people with frailty in acute care, formulating individualised management plans.
• Contribute to frailty pathway development, including ED frailty, inpatient liaison, and community integrated care pathways.
• Deliver high-quality person-centred care with a focus on safety, privacy and dignity, and support timely discharge to optimise patient flow.
