Job summary

Cardiff and Vale University Health Board is recruiting a Forensic Practitioner to support the community forensic team in assessment, intervention, and care co-ordination. The post requires working with a trauma-informed approach to reduce risk and improve outcomes for service users.

Job description

The Forensic Practitioner will support the delivery of specialist assessment, therapeutic intervention, and care co-ordination with the community forensic team. Key Responsibilities include: Clinical Responsibilities Utilise specialist forensic skills in consultation, assessment, intervention and care co-ordination. Deliver clinical care and promote evidence-based practice. Assist in assessment, formulation and care planning. Use critical thinking and clinical decision-making skills. Handle distressing situations sensitively, including managing complex situations. Ensure patient safety. Multi-Disciplinary Working Support therapeutic interventions within a multi-disciplinary team. Provide clinical support and advice to colleagues. Work autonomously and collaboratively within the team. Engage with service users and carers. Service Development Participate in training and research projects. Maintain high standards of documentation. Manage resources. Support managers with service development projects.,

We are seeking to appoint a skilled practitioner as a care coordinator for a community forensic caseload. You will be a practitioner who is able to work compassionately, understanding the impact of trauma whilst also managing risk of harm. Who is excited by the opportunity of working alongside professional colleagues in a dynamic, growing, and evolving Adult Community Forensic Service. The successful applicant will be a registered Mental Health Nurse, Learning Disability Nurse, Occupational Therapist or Social Worker and support the delivery of consultation, specialist assessment, therapeutic intervention, and care co-ordination within the Community Forensic Team. You will hold responsibility for delivering safe, effective, and evidenced care to support a community-to-community forensic pathway in the context of a trauma informed model of care.

• Professional qualification relevant to practice area, with study to degree level, with evidence of additional post graduate study
• Registered practitioner with professional body, Member of organisational group/network, locally or nationally
• Relevant forensic experience in a range of settings, Evidence of sound post-registration professional practice
• Evidence of continuing professional development
• Educated to degree level or be able to demonstrate equivalent knowledge or experiential learning/experience, Ability to demonstrate effective communication skills
• Ability to manage exposure to emotional/distressing circumstances
• Ability to demonstrate ethical values and attitudes within a culture of equality and diversity
