Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services across Whitland. This flexible, sessional role is ideal for ACPs with autonomous clinical assessment skills and experience in urgent patient demand management.

You will work with established clinical teams, delivering ACP-led sessions, handling acute and minor illness presentations, and providing safe prescribing within your scope of practice. A DBS check is required.

Qualifications

• Advanced Clinical Practitioner qualification (Level 7) required or equivalent.
• Experience in GP, urgent care or community health services preferred.
• Prescribing within scope of competence; DBS/clearance as applicable.

Responsibilities

• Deliver ACP-led sessions within community and primary care settings
• Manage acute, undifferentiated and minor illness presentations
• Support urgent patient demand through telephone or face-to-face triage
• Prescribe safely within scope of competence
• Manage clinical actions including results, medication reviews and follow-up tasks
• Record accurate and clear digital documentation
