Job summary

Wiltshire Council is hiring a First Responder Support Worker to assist with hospital discharges and to be the first point of contact for new customers. You will assess needs, plan support, and help with personal care and daily living activities across Wiltshire.

We offer a hybrid working model and opportunities for NVQ apprenticeships and upskilling through practice and skills development to nurture talent from within the council.

Responsibilities

• Support with hospital discharges and be first point of contact for new customers
• Assess customer needs, compile a support plan
• Assist with personal care, food preparation and activities of daily living
• Travel across the Wiltshire area to carry out customer visits
• Shifts include early morning, evenings, weekends and bank holidays
