Job summary

Meeveem Limited in Wolverhampton is seeking a fully HCPC- and BDA-registered dietitian to join a PCN on a fixed-term maternity cover, in-person role across several GP practices.

You will work as a senior clinician within the MDT, providing patient-centred assessment, diagnosis, treatment and evaluation, and supporting pathways of care.

Eligibility includes minimum 3 years of clinical dietetics, exposure to first contact dietitian pathways, and familiarity with EMIS/SystmOne is desirable.

Qualifications

• Fully HCPC and BDA registered practising Dietitian.
• Minimum 3 years of clinical experience in dietetics.
• Up-to-date understanding of Health Education England's Roadmap to Practice.

Responsibilities

• Assess, diagnose, triage, and manage patients, taking responsibility for a caseload.
• Develop and utilise full scope of clinical practice, including prescribing where trained.
• Manage complex cases with psychosocial needs; refer to social prescribing when appropriate.
• Deliver patient-centred interventions from initial assessment to outcome evaluation.
• Tailor care to individuals; assess health, well-being and employment impact.
• Support patients to set and achieve personal self-management goals.
