Job summary

Excellence Healthcare Services Ltd is seeking an experienced field care supervisor to join our domiciliary care team. Based in the community, you will supervise care delivery, conduct client assessments and reviews, and act as the link between service users, care workers and management.

The role includes supervision, quality monitoring, administering care plans, and maintaining records, with some weekend or out-of-hours care calls paid as agreed.

Job description

Excellence Healthcare Services Ltd is looking for an experienced, organised and compassionate field care supervisor to join our domiciliary care team.

This is a varied field and administrative role, providing an important link between our service users, care workers and management team.

The field care supervisor will spend a significant proportion of their working time in the community, monitoring the quality of care, carrying out staff spot checks and observations, conducting client assessments and reviews, supporting care workers and maintaining strong relationships with service users and their families.

The role also includes administrative responsibilities arising from field activities, including completing assessments, reviews, care documentation, quality records and ensuring that identified actions are appropriately followed up.

Key Responsibilities
Field Supervision & Quality Monitoring
  • Carry out regular and unannounced spot checks on care workers in service users' homes
  • Observe and monitor the quality of care being delivered
  • Monitor staff punctuality, professionalism, appearance, communication and adherence to care plans
  • Carry out staff competency observations, including medication, infection prevention and control, moving and handling and other relevant areas
  • Identify poor practice, performance concerns and training or development needs and escalation appropriately
  • Provide carers with constructive feedback, coaching and practical support
  • Support new care workers during induction, shadowing and their transition into independent community working
  • Assist with staff supervisions and performance monitoring as required
Client Assessments, Reviews & Relationships
  • Conduct initial assessments and home visits for new and prospective service users
  • Contribute to the development of person centred care plans and risk assessments
  • Carry out scheduled client reviews and reassessments
  • Review care arrangements when a service user's needs or circumstances change
  • Develop and maintain positive professional relationships with service users and their families
  • Obtain feedback from service users and relatives regarding the quality and consistency of care
  • Ensure care workers are following agreed care plans and respecting individual preferences
  • Identify changes in a service user's health, mobility, wellbeing or support needs and ensure concerns are appropriately escalated
  • Support the introduction of new care workers to service users where required
  • Respond professionally to concerns raised by service users or relatives and ensure appropriate follow up
Supporting Care Workers & Service Delivery
  • Act as an accessible point of support for care workers in the community
  • Attend service users' homes where concerns, incidents or care-delivery issues require supervisory input
  • Provide practical guidance to care workers dealing with changing or more complex situations
  • Support the safe implementation of new or amended care packages
  • Identify concerns regarding equipment, moving and handling arrangements or the home environment and report / elevate appropriately
  • Work closely with the care coordinator and management team regarding changes affecting service delivery
  • Assist with resolving day to day field issues promptly and professionally
Care Call Cover

The field care supervisor will form part of the operational contingency arrangements for the service.

Where necessary to maintain continuity of care, the field care supervisor may be required to cover care calls during their contracted working hours, for example where there is unexpected staff absence or an urgent need for cover.

Where the field care supervisor undertakes agreed care calls outside their contracted working hours, those additional hours will be paid separately at the applicable rate.

Administration & Documentation
  • Completing assessment and review documentation
  • Completing spot check and staff observation records
  • Updating care plans and risk assessments within their level of responsibility
  • Recording and following up actions identified during client visits and staff observations
  • Maintaining accurate records on the company's electronic care management systems
  • Reviewing relevant care records and electronic logs to identify quality or performance concerns
  • Completing incident, concern and escalation documentation where required
  • Maintaining accurate records of contact with service users, relatives and staff
  • Ensuring documentation is accurate, professional, timely and compliant with company procedures
  • Communicating outstanding actions to the appropriate member of the management team and following these through to completion
  • Undertaking other reasonable administrative and quality assurance duties associated with the role
Safeguarding, Compliance & Quality
  • Promote safe, compassionate and person centred care
  • Promote dignity, respect, choice and independence
  • Identify and promptly elevate safeguarding concerns
  • Monitor medication practices and report medication concerns or errors appropriately
  • Identify and report missed / late visits, unsafe practices and other quality concerns
  • Support internal audits and quality assurance activities
  • Assist with improvements arising from audits, complaints, incidents, service-user feedback and staff observations
  • Work in accordance with company policies, procedures, care plans and regulatory requirements
  • Maintain confidentiality and appropriate professional boundaries at all times
Working Hours & Flexibility

The position is 37.5 hours per week, normally Monday to Friday.

7am - 3pm with a 30 minute unpaid break.

Due to the nature of domiciliary care and the need to monitor care delivery at different times of the day, flexibility is an important requirement of this position.

On planned occasions, the field care supervisor may therefore be required to work an alternative shift, for example 11am - 7pm instead of 7am - 3pm.

Occasional alternative or weekend working may also be required for staff observations, quality monitoring, assessments or operational requirements. Reasonable advance notice will normally be provided wherever practicable.

The intention is not to increase the employee's normal contracted hours when an alternative shift is worked; rather, the working pattern may be adjusted to meet the needs of the service.

Essential Requirements
  • Have previous experience working within domiciliary / home care
  • Hold a full UK driving licence
  • Female only due to client preference
  • Have a good understanding of person-centred care and safe working practices
  • Have previous senior / supervisory experience or demonstrate the knowledge and capability required to undertake a supervisory position
  • Be confident conducting client assessments, reviews and staff observations
  • Have good written and verbal communication skills
  • Be able to produce accurate and professional care documentation
  • Be organised and able to balance field responsibilities with administrative duties
  • Be confident working independently and making appropriate decisions in the community
  • Be comfortable using electronic care-management systems and general office technology
  • Have access to their own reliable vehicle for work, as regular travel between service users and work locations is an essential part of the position
  • Already have the appropriate right to work in the UK without requiring sponsorship from Excellence Healthcare Services Ltd for this vacancy
  • Be willing to undertake an enhanced DBS check and all required recruitment checks
  • Be flexible in response to the operational requirements of a domiciliary care service
Desirable
  • Level 3 qualification in health & social care or equivalent
  • Previous experience as a field care supervisor, senior care worker or similar
  • Experience conducting care assessments and reviews
  • Experience completing care plans and risk assessments
  • Experience undertaking staff spot checks, supervisions and competency observations
  • Knowledge of medication management, safeguarding, moving and handling and infection prevention and control
  • Experience using electronic care planning and monitoring systems
  • Good understanding of CQC requirements and quality standards within domiciliary care
