Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Staff Nurse to join the Older People’s Mental Health Team in Newbury. The role covers community settings, involves travel to patient homes, and requires RMN registration with the NMC. You will assess needs, plan and deliver care, and work with carers and partner services from 9am–5pm, Monday to Friday.

Travel to Trust and non-Trust sites is required. A driving licence and access to a vehicle are essential.

Qualifications

• RMN qualification or PIN in progress.
• NMC registration is required.
• CPD evidence and ongoing professional development expected.

Responsibilities

• Manage a designated caseload in the community.
• Assess mental health needs across home visits and settings.
• Develop, implement and review care plans with patients and families.
• Deliver evidence-based interventions and monitor effectiveness.
• Coordinate care with primary care and partner agencies.
• Maintain accurate clinical records and comply with policies.
