Responsibilities
  • To provide comprehensive nursing / health assessments to determine the eligibility of clients for full NHS funded care.
  • Ongoing case management / coordination for clients receiving full NHS continuing health care funding and NHS fast track funding.
  • Provide a skilled and specialist comprehensive assessment of care needs for clients.
  • Process allocated applications for full NHS funded care in a concise and timely manner, ensuring a robust system is in place at all times.
  • Act as patient advocate for a complex and vulnerable client group.
  • Monitor and review care for own case load.
  • Undertake the provision of care plans where appropriate, in consultation with other professionals.
  • Ensure care is appropriately delivered by reassessing in all settings (hospital, nursing, residential or at home).
  • Liaise with acute sector, community services, social services, voluntary agencies as appropriate.
  • Work in partnership with patients and carers to enable them to make informed choices about care and treatment to promote their health and wellbeing.
  • Work in a collaborative manner with Health & Social Care colleagues in the coordination and best interest of the client.
  • Work in collaboration with the private sector Care Homes to facilitate their staff in accessing appropriate education opportunities.
  • Participate in research projects as required.
  • Participate in local and strategic health authority policy development within BNSSG.
  • Ensure that robust systems for communication within the team are followed.
  • Maintain clear, concise and up-to-date records of care, in line with approved standards.
  • Contribute to efficient budget management by optimizing the use of resources.
Role Overview

Working within the funded care team in the ICB, this fast‑paced and varied role provides the opportunity to use clinical judgement, analysis of information and heightened communication skills to support rapid access to NHS funded care. As part of a team of nurses, the successful candidate will process fast‑track applications, liaise with patients, their representatives and both community and acute trust sectors to ensure care needs are met and the patient is supported through their end‑of‑life journey.

The successful candidate will possess excellent communication and interpersonal skills, a good working knowledge of end‑of‑life care, the ability to problem‑solve, and make robust clinical decisions within ICB policy.
