Job summary

Cera is recruiting an Extra Care Scheme Team Leader to oversee care staff and ensure the welfare, independence and wellbeing of service users. You will lead care planning, monitor delivery, and coordinate with health services and families to deliver person-centred support in a housing-with-care setting.

The role requires rota planning, emergency cover, and ongoing supervision, with access to training and pension benefits. A strong focus on dignity and safety is essential.

Qualifications

• Candidates must have a relevant health and social care qualification.
• Experience with care planning and ensuring safe, dignified care.
• Knowledge of care inspections and quality standards.
• Experience with rota planning and scheduling.
• Experience supporting service users with additional needs.

Responsibilities

• Produce and review care plans for service users.
• Monitor care delivery to meet required standards.
• Update care plans and maintain accurate records.
• Provide direct care in emergencies when needed.
• Assist access to specialist care when appropriate.
• Encourage participation in daytime activities.
• Liaise with professionals, families and advocates.
• Administer and supervise medication per policies.
• Complete manual handling and risk assessments.
• Attend supervision, appraisals, and training.
• Lead a team to promote independence.
• Support on-call rota delivery; be designated OOH contact.
• Act as liaison with OOH teams during evenings/weekends.
