Job Summary

Woodhouse Health Centre is looking for an experienced Practice Nurse to join our team of Nurses and HCAs. The ideal candidate would be dynamic, able to work on their own initiative, committed, and a team player.

Main Duties
  • Deliver effective healthcare to patients, including long‑term condition management, wound management, vaccinations, and cervical cytology, depending on clinical competence.
  • Complete appropriate clinical coding in line with QOF and IIF targets.

Woodhouse Health Centre is one of the largest health centres in the area with around 12,000 patients. We cover a wide area and are an extremely busy practice with a large amount of vulnerable and elderly patients. We are also a GP training practice with six GP partners, salaried GPs, GP registrars and F2 doctors, and a variety of other clinical roles.

Job Description
  • Work as part of the Practice's multidisciplinary team and effectively deliver nursing services to the entitled patient population.
  • Provide routine nursing care to patients as required in accordance with clinical evidence, NICE and the NHS framework.
  • Prescribe medication in accordance with PGDs and own scope of practice.
  • Review patient medications to enhance compliance.
  • Oversee the provision of wound care services to patients.
  • Co‑ordinate travel medicine services.
  • Request pathology services as necessary.
  • Process pathology results as required.
  • Oversee chronic disease clinics, deliver patient care as necessary, and refer to secondary/specialist care as required.
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Ensure effective use of read codes.
  • Chaperone patients where necessary.
  • Prioritise health issues and intervene appropriately.
  • Support the team in dealing with clinical emergencies.
  • Recognise, assess and refer patients presenting with mental health needs.
  • Implement vaccination programmes for adults and children.
  • Support patients in the use of prescribed medicines or over‑the‑counter medicines (within own scope of practice).
  • Liaise with external services/agencies to ensure patients are supported appropriately (vulnerable patients, etc).
  • Delegate clinical responsibilities appropriately, ensuring safe practice and that tasks are within the scope of the individual.
  • First‑instance working pattern: Mondays 8:00 am‑5:30 pm (including a 30‑minute unpaid lunch break) and Thursdays 8:00 am‑1:00 pm, with potential for further hours.
Experience
  • Experience of working within a Nursing Team.

Qualifications

  • Postgraduate diploma or degree in Chronic Disease Management, Nurse prescriber.
Knowledge and Skills

  • Women’s health (cervical cytology, contraception, etc.).
  • Good working knowledge in Diabetes, Hypertension, Asthma, CHD, Immunisations (routine, childhood and travel).
  • Understand the importance of evidence‑based practice.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understand when to refer to GPs.
  • Understanding of infection prevention and control measures; understand the requirement for PGDs and associated policy.
  • Good interpersonal and communication skills.
  • Strong IT skills; clear, polite telephone manner.
  • Effective time management (planning and organising).
  • Ability to work as a team member and autonomously.
  • Ability to follow clinical policy and procedure.
  • Problem solver with the ability to process information accurately and effectively, interpreting data as required.
  • Ability to work under pressure/ in stressful situations.
  • Effectively able to communicate and understand the needs of the patient.
