Job Summary
The post-holder will be a qualified nurse, who is responsible for the delivery of practice nursing services care to the practice population. Supported by the Lead Nurse acting within their professional boundaries of their scope of practice, they will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will focus upon supporting patients to be healthy, monitoring and management of long‑term conditions, health prevention and screening activities, immunisations and may include cervical screening. The post-holder will work collaboratively with the multi‑disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. The post-holder will build effective working relationships with other health care professionals. The post-holder will work within the team of GPs, nurses and health care assistants to deliver excellent primary health care services within the Practice.

Job Description
Primary Responsibilities
- Chronic disease management including the diagnosis, monitoring and development of patient's individual management plans.
- Reviewing patients with chronic disease (diabetes, hypertension, heart disease) to implement and evaluate individual treatment plans to promote health and wellbeing.
- Assessing lifestyle and medications and suggesting changes where appropriate.
- Implementation of processes for the effective management of patients with long‑term conditions using evidence‑based practice.
- Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on patients' health.
- Venepuncture, handling of specimens.
- Undertaking diagnostic tests and screening procedures within agreed practice protocols (e.g. ECG and cervical cytology).
- Immunisation and vaccination of children, adults and travellers.
- Ear examination and syringing.
- Wound care, dressings and removal of sutures.
- First aid and management of emergencies (burns, haemorrhage, shock, cardiac arrest, triage of injuries).
- Care and maintenance of equipment.
- New patient checks.
- Provide appropriate signposting to the relevant clinician or external organisation whilst ensuring that the patient receives appropriate ongoing care.
- Maintain clear, concise and accurate documentation within the systems provided.
- Ensure that all necessary arrangements are completed promptly and in a safe manner.
- Maintain effective communication with multidisciplinary teams.
- Uphold the confidentiality of matters relating to patients and patient care in accordance with NMC guidelines.

- Foster good public relations by ensuring courtesy and discretion when dealing with patients and their relatives.
- Support and advise patients on the promotion of health, prevention of ill‑health and the self‑management of their health within their limitations.

- Ordering of supplies and equipment.
- Support junior members of the nursing team, providing guidance when necessary.
- Assist in screening procedures undertaken by the practice.
- Assist in the management of patients with chronic illnesses.

- Participate in the review of significant and near‑miss events applying a structured approach, i.e. root cause analysis (RCA).
- Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation.
