Job Summary
We are looking for either one full time or 2 part time Practice Nurses to work as part of the practice nursing team to provide effective nursing care, according to practice policy, using a range of clinical skills. The role involves undertaking the delivery of nursing care for patients as a self‑directing practitioner, working with the Partners to meet medical needs as appropriate.

Job Responsibilities
- Provide care for minor injuries, dressings, including pressure bandaging and higher level wound care, liaising directly with community and secondary care where appropriate and escalating to a GP as necessary, and supervising less qualified nursing colleagues in the practice.
- Perform cervical cytology sampling, immunisations as outlined by national programmes and local guidelines, and promote the service to hard‑to‑reach patients.
- Support the annual flu campaign and other mass population vaccination programmes as needed in line with national guidance.
- Conduct and assist GPs in providing LARC services and assist in the provision of minor procedures.
- Act as a chaperone or similar support for other colleagues as needed.
- Supervise other colleagues in anthropometric measurements, blood pressure, spirometry, urinalysis, phlebotomy, microbiology samples, ECG, ABPV, peak flow and other practice‑based tests as needed.
- Provide patient health promotion/disease prevention education and counselling focusing on health behaviour change.
- Provide a comprehensive travel advice service and travel immunisations to patients in accordance with practice protocols.
- Triage patients in person or over the phone to the appropriate member of clinical staff to meet a patient’s stated needs.
- Assist members of the clinical team in providing a seamless service through an illness episode, directing the nursing component of any illness episode.
- Liaise with community nursing colleagues to continue nursing care in the home if patients cannot attend the practice, and transfer or share care as appropriate.
- Support GPs in the management of chronic diseases such as diabetes, COPD, asthma, hypertension, etc.
- Promote the health and wellbeing of patients by counselling on conditions, medications, diet, exercise and other health behaviours.
- Provide support and guidance to patients in times of crisis, breaking bad news where necessary, and provide ongoing support.
- Advise patients on the meaning of test results and other medical information, conduct phone and in‑person consultations, and triage to appropriate clinicians or arrange additional investigations as needed.
- Participate in a robust system of clinical audit, infection control and medicines management, and support clinical governance.
- Maintain accurate patient records, enter appropriate agreed codes, use agreed standards of data entry, templates, and protocols, complete all documentation as required for good patient care.
- Support the SPN in creation, maintenance, documentation and training of others in robust systems of data collection, audit and review of all clinical activities, protocols, care pathways and clinical systems.
- Create and maintain a system of clinical stock and equipment maintenance, calibration, and supply; work closely with the Practice Manager and SPN to ensure cost‑efficient purchasing.

- Participate in infection control procedures and protocols and their implementation within the practice.
- Maintain your own annual registration with the NMC and adhere to its Code of Professional Conduct.
- Maintain any other required registrations and certifications necessary to fulfil the role.
- Promote a professional approach at all times by dress and attitude to patients and staff, setting an example for those under your direct authority and the team as a whole.
- Maintain your own continuous CPD and keep a PDP up to date in line with the practice and nursing team development plans.
- Participate in and attend practice meetings and in‑house training.
- Provide constructive feedback to all members of the team and challenge colleagues when necessary to maintain high standards of professionalism, patient care, service and excellence.
Person Specification
Essential
- GCSEs (or equivalent) grade A to C in English and Maths
- Registered Nurse with current NMC registration
Desirable
- Ability to undertake and interpret clinical measurements such as blood pressure, spirometry and phlebotomy
- Proven experience working as a practice nurse in a primary care setting
- Competence in clinical skills including wound care, immunisations, cervical cytology and chronic disease management
- Knowledge of national health programmes and immunisation schedules
- Strong communication and interpersonal skills for patient education and team collaboration
- Ability to work independently and as part of a multidisciplinary team
- Basic IT skills and ability to maintain accurate electronic patient records
- Understanding of infection control procedures and clinical governance
