One Ellesmere Port Primary Care Network (PCN) delivers a wide range of innovative, community-focused services designed to improve health and wellbeing across our town. Services include medicines optimisation, mental health and wellbeing support, first contact physiotherapy, social prescribing and counselling, vaccination clinics, and proactive care coordination for vulnerable patients.

Job Responsibilities
- Deliver high‑quality, evidence‑based nursing care to patients across the PCN.
- Manage long‑term conditions, prioritising asthma and COPD.
- Lead long‑term condition management initiatives and support quality improvement.
- Support health promotion and public health screening programmes, including immunisations and cervical screening.
- Embed population health management approaches to target health inequalities and improve access.
- Contribute to the delivery of national and local enhanced services.
- Support, mentor, and supervise new‑to‑practice nurses and other healthcare professionals within the PCN.
- Work collaboratively across the PCN to enhance integration with community, secondary, and social care services.
Person Specification – Essential Experience
- Working experience in a primary care or PCN environment.
- Experience of long‑term condition management.
- Experience of working autonomously.
- Experience of infection prevention and control measures.
- Experience of quality improvement initiatives (benchmarking).
Person Specification – Desirable Experience
- Experience as a practice nurse or community nurse.
- Experience of managing elderly and housebound patients.

- Meet the requirements for nurse revalidation.
- Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse.
Qualifications – Essential
- Registered Nurse (Nursing and Midwifery Council).
- Level 6 diploma or postgraduate certification in long‑term conditions care and/or public health initiatives.

- Wound care – removal of sutures and staples.
- ECGs.

- New patient medicals.
- Requesting pathology tests and processing the results, advising patients accordingly.
- Understanding the importance of evidence‑based practice.
- Understanding the requirement for PGDs and associated policy.
- Ability to record accurate clinical notes.
- Ability to work within own scope of practice and understanding when to refer to GPs.
- Chaperone procedure.
- Women’s health (cervical cytology, contraception, etc.).
