Specialist Carers - Reablement & Intermediate Services

Are you a skilled care professional ready to support people in regaining independence after hospital or during periods of intermediate care? At First City Care Group, our Reablement and Intermediate Services teams deliver specialist, person-centred care that transforms lives across Swindon.

Why This Role is Different:
  • Work with clients transitioning from hospital and/or receiving intermediate care, supporting recovery and preventing unnecessary hospital stays.
  • Collaborate closely with occupational therapists, nurses, social workers, and other professionals to design and deliver tailored care plans.
  • Deliver short-term, outcome-focused interventions that empower clients to regain confidence, mobility, and independence.
Your Responsibilities:
  • Provide high-quality, person-centred care, including personal care, medication, mobility support, meal preparation, and light household tasks.
  • Support clients in rebuilding skills and confidence, promoting independence at every stage of recovery.
  • Assess changing care needs in collaboration with the wider health and social care team.
  • Mentor junior staff and contribute to a culture of professional excellence and continuous improvement.
Who We're Looking For:
  • Minimum 1 years' experience in health & social care
  • NVQ Level 3 (or equivalent) in Health & Social Care (preference)
  • Full UK driving licence and access to a reliable vehicle
  • Strong communication skills and ability to work collaboratively with professionals across multiple disciplines
