A leading mental health charity in the UK is seeking an Enhanced Care Recovery Practitioner to provide flexible outreach support to service users with multiple needs. This role involves assessing and engaging individuals, managing caseloads, and conducting home visits. The ideal candidate will have a strong passion for making a difference, knowledge of effective intervention techniques, and must hold a driving licence. The position offers a range of benefits, including ample annual leave and a pension scheme.

Working Hours: 37 hours, Monday-Friday
Contract Type: Permanent
Salary: £25,110 - £32,090 per annum

As an Enhanced Care team Recovery Coordinator, you will be employed by Waythrough to support higher need service users who are unable to progress their timeframe due to their multiple-needs. Multiple needs include but not limited to; chronic physical health, comorbidities, financial issues, memory/cognitive impairment, age, housing, limited social support and other multi-agency support needs.
This is an interesting and rewarding role where you will have the flexibility to complete greater levels of outreach and go to service users to reduce barriers to engagement, support them to engage with specialist services, bring the intensive support and interventions to the individuals and enhance positive outcomes through this flexible working.
