Overview

Are you an experienced and compassionate Emergency Care Practitioner/Advanced Nurse Practitioner, seeking an opportunity to make a lasting impact on patient care? Join our dedicated team at HCRG Care Group, based at Livingstone Hospital in Dartford. This is a chance to work in a supportive, forward-thinking environment where your clinical expertise, autonomy, and leadership will be highly valued. In this role you will play a vital part in delivering high-quality, patient-centered care to our diverse community. Working within a multidisciplinary team your experience in diagnosing, managing, and treating a wide range of acute and chronic conditions will be essential in ensuring positive health outcomes for our patients within the North Kent community. You will support the delivery of care to a range of complex patients which will include: life limiting conditions like diabetes, in dwelling long term catheter, urgent wound care etc. This role covers both Dartford and Gravesend, you may need to work in both Livingstone Community Hospital and Gravesham Community Hospital.

Responsibilities
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients to support compliance with and adherence to prescribed treatments
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions
  • Strategic qualification items and registration: Recognised Nursing Degree Qualification (or equivalent); Registered with the NMC; Masters in relevant area or working towards; Independent Nurse Prescriber
