Position

An exciting opportunity has arisen for a Team Leader within our enhanced Intermediate Care Team based at Ayrshire Central Hospital, Irvine.

The Enhanced Intermediate Care Team (eICT) is a multidisciplinary team (MDT), working together with patients and carers to provide rapid assessment and rehabilitation for our service users. eICT offers an alternative to avoidable hospital admission by maintaining people in their homes and to facilitate the smoothest and earliest possible discharge from hospital. The multi-disciplinary team provides assessment, short term intensive rehabilitation and support. The ability to utilize advanced communication skills as part of a wider MDT is vital along with knowledge of Older Peoples Services, Care Management and the rehabilitation Framework is essential along with a genuine interest and understanding of inter-professional and inter‑agency working.

What you will do:

The team leader will play a critical part in leading the team to provide high quality and effective care. You will lead and develop eICT, which through assessment, rehabilitation and care management; provide an alternative to hospital admission and facilitate early hospital discharge. The team will achieve that by assessing, planning, implementing care and managing patients from hospital, community and other settings. You will lead eICT and take forward partnership objectives ensuring compliance with appropriate clinical, financial, staff and corporate governance objectives and reporting arrangements.

Knowledge, training and/or experience required to do the job:

A diploma/degree in a relevant clinical profession is essential with evidence of working autonomously within the community setting. You should also have extensive clinical experience and evidence of continuing professional development. You will have the opportunity to develop competencies in leadership and management, working in an inter‑professional manner.

Hours and Shift Pattern:

The service delivers care Monday to Friday and has a limited service at weekends. The team leader will generally work Monday – Friday however some weekend working may occur. This post is offered on a permanent basis.

Driving Licence/Car Ownership:

Full UK driving licence and access to a vehicle is essential to this role.
