Job description

We are looking for qualified and experienced Early Intervention & Prevention Practitioner to join our exceptional Early Intervention & PreventionTeam.

What will be expected from me?

The purpose of this role is to work as part of a multi-disciplinary team to support the delivery of the Early Intervention and Prevention offer for families, adopting a whole family, strengths-based approach to reduce risk, increase resilience and improve outcomes for children.

What I might be doing?
  • You will deliver and lead core functions of the Wakefield Families Together Family Hub delivery model, working with partner agencies to improve outcomes for children, young people, and families.
  • You will be responsible for provision of statutory returns and information to Government and other agencies in relation to adult social care, in line with statutory requirements.
  • You will facilitate the development and implementation of an agreed plan of support for children and families through the Team Around model.
  • You will act as a ‘duty worker’ and will often be the first person who meets and speaks to families helping them think about the best way that our service, or other community services can help them.
  • You will ensure that services are delivered in compliance with relevant legislation, local and national policy, and guidance.

Essential requirements:
  • Recognised Level 5 or Degree Level qualification in a relevant subject area, or the ability to demonstrate an equivalent level of knowledge and skill gained through relevant experience and employment.
  • An understanding of the risks and needs that children and young people experience.
  • An understanding of the principles that support and promote the wellbeing of children, young people, and families.
  • Experience of working directly with children, young people and families with complex needs and risk in a relevant setting.
  • Skilled in undertaking assessments of need and risk for individuals, activities or groupwork interventions. Using assessment tools where appropriate.
