Qualifications

• A Social Work Qualification or equivalent.
• Understanding of relevant legislation.
• Relevant Social Work experience.

Responsibilities

• Prepare for social work contact and involvement.
• Assess support needs for Service Users, Carers, and Families.
• Produce, deliver, and update Care & Support Plans.
• Work with Service Users, Carers, and Families to assist decision-making.
• Be proactive with respect to Safeguarding issues.
• Identify, assess, and manage risks to Service Users.
• Respond to crisis situations with appropriate legal intervention.
• Attend and participate in meetings and decision-making forums.
• Provide information, reports, and statistics as needed.
• Develop and maintain relationships with partnership agencies.
• Participate in formal and reflective supervision sessions.
• Inform managers of service deficiencies or issues.
• Attend training and development activities.
