A community health service provider in Cambridgeshire is seeking a medical professional for their Community Paediatric Team. The role involves conducting health assessments for children in care, formulating individual care plans, and collaborating on multi-agency audits. To work within the Community Paediatric Team to understand and assess the health needs of children and young people who are looked after, including those with a plan for adoption. The role focuses on initial health assessments for children in care and contributes to developmental assessments across Cambridgeshire.
Good communication skills to allow effective teamwork with clinical and non‑clinical staff in health, education and social care.
Maintain personal professional development, participate in appraisal and KSF processes, undertake mandatory training and other relevant training as required.
Adhere to confidentiality, data protection, Trust information governance and security policies, risk assessment processes and infection control policies. Participation in clinical and other audits, emergency preparedness processes, and observation of patient safety standards are required.
