This is an exciting opportunity for a motivated and passionate Registered Nurseto play a key role in shaping high‑quality community nursing servicesand making a real difference to patient care.
The post holder will take an active role in monitoring and improving the quality of care, identifying opportunities for innovation, and leading on service development to meet the evolving needs of the local population.
You will be supported to hold or work towards the District Nurse Specialist Practitioner Qualification (DNSPQ), with opportunities to develop professionally within a forward‑thinking service.
Working as part of a supportive, integrated multidisciplinary team, you will provide clinical leadershipacross the community nursing caseload through proactive and effective caseload management. You will have the autonomy to work independently while also being a visible, approachable leader—supporting, guiding, and developing members of the nursing team to consistently deliver high‑quality, evidence‑based care.
The role offers the chance to build strong partnerships with health, social care, and voluntary sector colleagues, ensuring coordinated, person‑centred care and positive patient experiences. This position is ideal for a nurse who is enthusiastic about community practice, values teamwork, and is keen to influence service delivery while developing their leadership skills.
The post holder will monitor the quality of care provision and identify and lead on areas of service development.
They will be a registered nurse who holds or working towards the DNSPQ qualification.
They will work within an integrated team providing clinical leadership for all patients within the community nursing caseload through proactive management. Whilst working on their own initiative they will be responsible for ensuring that all members of the nursing team are providing the highest standard of evidence based clinical care and will actively support and liaise with other health and social agencies and professionals.
