District Nurse / Caseload Holder – Band 6

Out of Hours Service 18.000 – 23.00
10 hours a week one shift

Overview

We are transforming Adult Community Services in Croydon. The role is part of an Integrated Care Team providing efficient and effective nursing care to patients in their own homes.

Responsibilities
  • Provide district nursing care to clients in their own homes.
  • Assess, plan and deliver care for clients referred to the service.
  • Promote evidence‑based practice for pressure ulcer prevention and management.
  • Collaborate with the tissue viability team and other health professionals.
  • Offer education and support to patients, carers and other health care professionals.
  • Supervise the delivery of care by clinical staff.
Qualifications
  • Registered Nurse with a Specialist Practice Qualification in District Nursing or similar.
  • Clean full UK driving licence and access to a car for work purposes.
  • Excellent clinical, leadership and communication skills.
  • Experience in community nursing or integrated community networks is desirable.
