Job summary

Mid and South Essex NHS Foundation Trust is seeking an Integrated Discharge Team Facilitator to work across Brentwood Community Hospital and other sites in Basildon. The role is full-time, 37.5 hours, with a pattern covering seven days.

You will support discharge planning, collaborate with multidisciplinary teams, and help implement care plans while ensuring adherence to care act and regulatory requirements. A full UK driving licence and access to a vehicle are required.

Qualifications

• Previous Administrative Experience
• Experience coordinating discharges or complex patient pathways
• Previous NHS experience
• GCSE qualifications at minimum
• Knowledge of hospital discharge process

Responsibilities

• Deliver high-quality, clinically effective and holistic care through collaboration with multidisciplinary colleagues
• Assume accountability and responsibility for own decisions
• Support and training to healthcare assistants with IDT Nurse Assessors
• Contribute to continuous improvement of standards of care
• Develop care plans for patients referred to the Integrated Discharge Team
• Provide advice and instruction to patients and carers to promote understanding of discharge plans
• Prioritise own caseload and facilitate IDT Assessors
• Educate patients and carers to participate effectively in discharge planning
