Job summary

LinsCare is seeking an experienced Occupational Therapist to join the Adult Social Care Hospital Discharge Team in London. You will conduct specialist OT assessments, identify functional needs and risks, and recommend equipment and adaptations to enable safe discharge.

Working with multidisciplinary colleagues, you will support timely discharge, reduce delays and promote independence for adults with health and social care needs. HCPC registration is required.

Qualifications

• Recognised qualification in Occupational Therapy.
• HCPC registration with current status.

Responsibilities

• Undertake comprehensive occupational therapy assessments in accordance with the Care Act 2014 and professional standards.
• Complete strengths-based assessments focusing on independence and outcomes.
• Assess functional abilities, mobility, transfers and daily living risks.
• Identify equipment, assistive technology and home adaptations to support discharge.
• Undertake moving and handling assessments and manage risks.
• Develop outcome-focused plans to maximise independence and reduce long-term care needs.
• Support individuals to regain function after hospital admission.
• Facilitate safe, timely discharges from hospitals and contribute to Discharge to Assess pathways.
• Collaborate with NHS staff, social workers and care providers in discharge planning.
