Job description
Job Overview

The post holder will support the Integrated Discharge Team and Enhanced Care Team, working with ward staff, therapies and external partners to facilitate discharge flow and organise Enhanced Care services. Outputs will meet Patient Safety, Information Governance and CQC recommendations.

Key Responsibilities
  • Maintain discharge flow and update the Patient Discharge Optimisation Tool (OPTICA).
  • Schedule transport for discharge for all patients across the trust.
  • Use the trust electronic systems (Trakcare, SystmOne) to access patient details and input information onto active clinical notes.
  • Support wards with paperwork and ensure compliance with Trust KPI’s.
  • Build strong communication with the multidisciplinary team to keep patient information up to date.
  • Address discrimination, promote equality/diversity and zero tolerance on violence.
  • Establish good working relationships with hospital staff and local authority partners.
  • Deal with service user queries consistently with legislation, policies and procedures.
  • Assist in planning and prioritising workload to meet service priorities.
  • Data entry and retrieval of results using available technologies.
  • Provide interim short‑term cover for sickness absence/annual leave.
  • Maintain a healthy, safe and secure working environment.
  • Support enhanced care caseload when applicable.
  • Take responsibility for own education/training.
Qualifications

Essential criteria

  • Good working knowledge of Windows‑based computer systems and Microsoft Office applications (Excel, Word, Outlook).
  • Ability to work under pressure, meet tight deadlines and deliver high quality work.
  • Excellent communication, organisation skills and ability to work with a multidisciplinary team.

Desirable criteria

  • Experience dealing with members of the public.
  • Traccare knowledge and understanding.
  • Experience working within a healthcare environment and within a multi‑disciplinary team.
