A leading healthcare provider in the UK is seeking skilled professionals to manage patient caseloads and provide high-quality care and assessment. The position requires strong organisational skills and a commitment to patient dignity and confidentiality. Candidates must be a Registered Nurse, Physiotherapist, or Occupational Therapist with significant experience. This role plays a key role in training junior staff and ensuring effective patient discharge planning.
TheJobholder is responsible for the management of a defined caseload and carries out continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.
Thejobholder will provide specialist assessment care and advice to a specific patient group and will contribute to the development of the speciality service and will ensure cost effective use of resources.
Current services comprise providing specialist advice, support and training to ensure high quality discharge planning is undertaken by ward staff.
Fulfil the requirements as directed by the Nursing and Midwifery Council or HealthCare Professions Council to maintain and revalidate registration.
Provide a clinical lead and role model to clinical staff by delivering high quality care.
Support patients and their families, communicating sensitive condition related information and ensuring they receive required information to enable them to participate in their care delivery.
