A healthcare organization in Greater London seeks a case manager responsible for facilitating safe patient discharges. This role involves coordinating with multidisciplinary teams, overseeing discharge navigators, and maintaining momentum in the discharge planning process. Candidates should possess critical thinking and effective communication skills, ensuring patients are informed and supported throughout their discharge journey. The post holder will be responsible for case managing and facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate and oversee Discharge Navigators within their sub‑team, complete assessments of patients' needs and support the discharge plan. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Transfer of Care Hub Team (TOCH) to overcome and limit these. The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge.
