Job summary

A leading healthcare provider in Poole is seeking a Discharge Coordinator to support and manage complex discharge processes for patients in an acute ward. The ideal candidate will have qualifications in health/social care management and a strong understanding of patient flow. Experience in a hospital setting is essential. This role offers the opportunity to work within a dedicated team committed to quality patient care.

Qualifications

  • GCSE / A level or equivalent standard in numeracy and literacy.
  • NVQ level 3 in Health/Social care management.
  • Experience in patient care in acute settings.

Responsibilities

  • Support complex discharge processes in an acute ward area.
  • Plan patient discharge requiring CHC and Social services involvement.
  • Assess patient needs within acute care and community settings.

Person Specification
Qualifications
  • Numeracy and Literacy-GCSE / A level or equivalent standard
  • NVQ level 3: Health/ Social care management or equivalent.
Experience
  • Understanding of patient flow and supporting complex discharge processes in an acute ward area.
  • Experience of caring for patients with acute and long term conditions in a hospital setting
  • Demonstrable clinical experience
  • Experience of supporting others with the process of planning patient discharge requiring CHC and Social services involvement.
  • Experience of patient flow and discharge planning systems
  • Basic IT skills
  • Assessment, planning and organisational skills.
  • Awareness of audits
  • Good knowledge of NHS computer systems, e.g Ecamis, EPR
Knowledge
  • Demonstrated knowledge of both simple and complex discharge form an acute ward, including mental capacity act
  • Understanding of National Frameworks, CHC, Delayed Discharge Act and reimbursement
  • Understanding of MDT assessment and treatment for vulnerable patients within acute care and community settings.
  • Knowledge of nation guidance and target relating to discharge
