Qualifications

• Registered with Social Work England.
• Must hold a full UK driving licence.
• Minimum of post-qualified experience in social work.

Responsibilities

• Complete holistic disability assessments for individuals with lifelong disabilities.
• Manage safeguarding concerns and coordinate risk-management plans.
• Develop outcome-focused support plans involving multi-agency services.
