Overview

A fantastic opportunity has arisen for a Diabetes Specialist Nurse to join our Cambridge Team working full time 37.5 hours per week. If you have compassion, empathy with the ability to work autonomously, this is the opportunity for you.

You will work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for patients with Diabetes disease within the community.

Responsibilities
  • Manage a caseload of patients with complex diabetes needs, using evidence based practice and patient centred goals to assess, plan, implement and evaluate.
  • Promote better understanding of diabetes for patients, their families and carers through education, support and counselling.
  • Act as key worker, coordinating multidisciplinary care.
  • Provide expert support, advice and education to raise awareness of the needs of patients with diabetes.
  • Undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues.
  • Work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team.
  • Deliver accredited Type 2 (DESMOND) Structured Education to newly diagnosed patients.
  • Attend regular team meetings and other meetings as required.
Qualifications
  • Plan, implement and evaluate individualised specialist patient care programmes based on evidence based practice and the application of clinical reasoning skills.
  • Promote and support self‑management and engage with patient support groups.
  • Be professionally and legally accountable for all aspects of own work including management of patients in your care.
  • Organise the caseload efficiently and effectively with regard to clinical priorities and use of time.
  • Recognise and act on signs, symptoms and complications of diabetes conditions.
  • Hold a full UK driving licence and have use of a vehicle; public transport is not acceptable.
