Job summary

Calderdale and Huddersfield NHS Foundation Trust is seeking an experienced Lead Diabetes Specialist Nurse to support the Diabetes Nursing Team. You will lead service development projects and contribute to patient-centred care improvements across inpatient and outpatient settings.

The post holder will deliver Level 4/5 diabetes services, manage staff rosters, appraisals and sickness, and work closely with the broader team to drive quality improvement within CHFT.

Qualifications

• Advanced diabetes knowledge with experience caring for Type 1 and Type 2 diabetes in acute or primary care.
• Autonomous practitioner able to deliver level 4 and level 5 diabetes service, including inpatient and outpatient care.
• Strong leadership and people management skills, with a track record of service development.

Job description

The post holder will support the Lead DSN in leading the Diabetes Nursing team and service, demonstrating enthusiasm and a commitment to further developing their knowledge and skills. They will share the team's vision and be proactive in contributing to service development projects aimed at improving patient care.

You will have specific experience of clinical leadership, possess enthusiasm and determination to improve the quality of patient centred care, service delivery and service team projects.

You will have experience of managing and developing a team, completing staff appraisals, sickness monitoring and roster management.

The successful candidate must possess advanced diabetes knowledge and experience looking after people with Type 1 and Type 2 diabetes, either in an acute or primary care setting.

The post holder will be an autonomous practitioner able to deliver level 4 and level 5 diabetes service, including both inpatient and outpatient care.

You will be self-motivated, organised and capable of adapting to the changing needs of the service. You will demonstrate excellent interpersonal skills, be computer literate and preferably familiar using SystmOne and EPR.

The post holder will work as an integral part of the established specialist Diabetes Nursing Team as the diabetes lead for Calderdale and Huddersfield NHS trust.
