A healthcare organization in Bradford is seeking a dedicated diabetes nurse to provide clinical support in community and specialist clinics. You will manage a caseload of diabetes patients, ensuring high-quality care and participating in education sessions. The ideal candidate will have significant clinical experience, NMC registration, and strong communication skills. The post holder will work as part of the diabetes integrated multidisciplinary team providing clinical support to community based intermediate diabetes care clinics, specialist diabetes clinics, and the in-patient population with diabetes. The post-holder will be responsible for the management of a defined caseload of patients with diabetes, providing specialist nursing support, advice, education and/or care to that specific group. They will also participate in the delivery of structured patient diabetes education sessions and healthcare professional education for community and hospital-based staff. Clinical care will be delivered at numerous primary and secondary care locations across Bradford.
The post holder will work as part of an innovative diabetes nursing team, supporting the implementation of Think Glucose, a national initiative designed to improve the care of in-patients with diabetes, across Bradford Teaching Hospitals Foundation Trust. They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within BTHFT.
The diabetes specialist team delivers a range of patient‑oriented structured education sessions of which many have been accredited by a national scheme (Qismet); the post holder will take an active part in this service.
