A healthcare institution in Bradford is looking for a diabetes nurse to provide clinical support in community-based clinics and hospitals. Responsibilities include managing a caseload of diabetes patients, delivering patient education, and working as part of a multidisciplinary team to improve patient care. The post holder will work as part of the diabetes integrated multi-disciplinary team providing clinical support to community based intermediate diabetes care clinics, specialist diabetes clinics, and the in-patient population with diabetes. The post-holder will be responsible for the management of a defined caseload of patients with Diabetes, providing specialist nursing support, advice, education and/or care to that specific group. They will also participate in the delivery of structured patient diabetes education sessions and healthcare professional education both for community and hospital-based staff. Clinical care will be delivered at numerous primary and secondary care locations across Bradford. The post holder will work as part of innovative diabetes nursing team, supporting the implementation of Think Glucose, a national initiative designed to improve the care of in-patients with diabetes, across Bradford Teaching Hospitals Foundation Trust (BTHFT) They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within BTHFT.
The diabetes specialist team delivers a range of patient orientated structured education sessions of which many have been accredited by a national scheme (Qismet) and the post holder will take an active part in this service. The post holder will take daily responsibility for managing a defined caseload, with the support of existing nursing and medical staff. They will utilise skills to assess and identify patient needs and implement delivery of appropriate individualised care and treatment for people with diabetes.
