Job Summary

We are looking for a Practice Nurse with a special interest in Diabetes to join our friendly, proactive dispensing practice for 10 hours a week.

About Us

We have an excellent, dedicated and loyal team working from purpose‑built premises with a patient list of approximately 6,000. Our practice uses TPP SystmOne and works closely with 7 GPs, 3 PNs and 3 HCAs.

Job Description

Job Title: Diabetic Nurse
Reports to: Partners/Lead Nurse (clinical) and Practice Manager (administrative)
Hours: 10 hours per week (part‑time, permanent contract)

Key Responsibilities:

  • Provide diabetic nursing support to patients independently and in partnership with nurse colleagues and general practitioners.
  • Maintain required training and competence, working within patient group directions where available.

Main Duties:

  • Management of patients with diabetes.
  • Diagnosis, monitoring, and development of individual management plans, agreeing with patients and other health professionals.
  • Therapeutic monitoring, checking compliance with treatments and identifying abnormalities.
  • Recommend and implement necessary changes to treatment plans.
  • Attend patients at home if necessary.

Supporting responsibilities include:

  • Maintaining confidentiality of patient information.
  • Promoting health and safety according to the Practice Health & Safety Policy.
  • Supporting equality, diversity, and inclusion in all interactions.
  • Engaging in personal and professional development activities.

Quality and Communication:

  • Alert and mitigate quality and risk issues.
  • Assess own performance and collaborate with the team to improve processes.
